Cancellations

Please call the office 24 hours in advance if you need to cancel an appointment.

Late Policy

We are committed to prompt service and will work diligently to stay on time. We may ask you to reschedule if you arrive more than 15 minutes late to your appointment. If you are a new patient, have new insurance or need to complete any paperwork associated with your visit, please arrive 30 minutes early.

Referrals

Please call the office at least three (3) days before the appointment to complete the information needed for the referral. That information includes:

  1. Your name
  2. The physician you are being referred to
  3. His or her phone and fax numbers
  4. The date of the appointment
  5. The diagnosis for the referral
  6. Your insurance
  7. Whether the referral is for a visit or a specific procedure (such as surgery)

We do not mail referrals to specialists.

Lab and X-Ray Appointments

If your physician has ordered a lab or x-ray, please call the office at least 24 hours prior so we may have your paperwork ready when you arrive.

Prescriptions

If you need a prescription refill please have your pharmacy fax us a request.

Emergencies and On Call

If you have an emergency, such as chest pain, severe shortness of breath or bleeding, call 911 or proceed directly to the nearest hospital emergency room.
